Welcome to my lighter and brighter area for processing forms!
Please copy and paste the following information into a new email form or blank Word document. Type the appropriate responses for each question. Delete any sections that don't apply to you. There's no law that says you have to do the garter rituals or the dollar dance. It's all up to you. As with the intake form, I prefer email communication so there's always a record of arrangements, including the changes that brides tend to make along the way. I don't like PDF forms because they don't allow me to highlight, copy and paste information into other documents.

to cover announcement of all names, a second song won't be needed.
It may disrupt the flow of announcements, or it may be cut off at an inopportune

Choose one of the following styles to announce the bride and groom.

--Jenny and John (Common for brides who like a relaxed and familiar style.)
--Jennifer and Jonathon Sharp (The most standard option for weddings.)
--Mr. and Mrs. Jonathon Sharp (Some feel this traditional style negates the woman.)
--Dr. and Mrs. David Sharp (Common for medical doctors and college professors.)

TRADITIONAL WEDDING DANCES
At this time, I will ask you to help me gather the following people and position
them beside the dance floor. It's very important to line up the dancers in one place
to avoid announcing dancers who might be in the bathroom or lobby. We should
also visit the courtyard to ensure that all smokers are called inside for the rituals.
Song suggestions are provided to help with the brainstorming process,
but I have no preference. Insert any song you like!

Songs of friendship may be desirable if your paired dancers aren't necessarily lovers.
Intensely romantic songs can sometimes make wedding party dancers look uncomfortable.
For your convenience, I've provided a quick list of friendship songs. You are not obligated

OPEN FORMAT DANCING FOR THE GUESTS
Enter a response of A, B or C to indicate which styles of dance music you like best.
Choose as many categories as you like. I'll avoid playing music from categories you don't choose.
A. JAZZ & SWING: (Count Basie, Duke Ellington, The Brian Setzer Orchestra, etc.)
B. HIGH ENERGY OLDIES: (Elvis, The Beach Boys, Chuck Berry, Jerry Lee Lewis, Buddy Holly.)
C. MOTOWN & SOUL: (The Temptations, The Four Tops, Stevie Wonder, Smokey Robinson,
Jackson 5, Al Greene, Diana Ross, Marvin Gaye, Tammi Terrell, Mary Wells, The Ronettes.)
D. DISCO & FUNK: (The Bee Gees, Donna Summer, KC & The Sunshine Band, Rick James,
Teena Marie, Village People, The Gap Band, Parliament, Earth, Wind & Fire, Barry White.)
E.1980s DANCE MUSIC: (Prince, Michael Jackson, Madonna, The Pointer Sisters, Lionel Richie,
George Michael, Chakua Khan, Whitney Houston, Janet Jackson, Taylor Dane, Mary Jane Girls.)
F. OLD SCHOOL HIP HOP: (The Beastie Boys, Salt-N-Pepa, Run D.M.C., Rob Base, Tone Loc,
LL Cool J., 2 Live Crew, Digital Underground, Young MC, MC Hammer, Bell Bib DeVoe.)
G. Modern Mainstream Pop & Hip Hop: (Justin Timberlake, Jason Derulo, Pharrell Williams,
Mark Ronson, Usher, The Black Eyed Peas, Taylor Swift, Beyonce, Rihanna, Katy Perry, Lady Gaga,
Kesha, Nicki Minaj, Mary J. Blige, Meghan Trainor, Ne-Yo, Chris Brown, Akon, Kanye West.)
H. Rap with an Edge: Eminem, Tupac, T.I., Lil Wayne, Gucci Mane, 36 Mafia, DMX
I. TECHNO & FREESTYLE: Rockell, Lil Suzy, Debbie Deb, La Bouche, The Real McCoy
TIP: Photographers often leave after the garter rituals, and senior citizens often leave early.
For beautiful family dance floor photos, it's vital to lure the parents and grandparents
to the dance floor early in the evening. Nothing gets older people dancing like the hits from
categories B & C.
STANDARD LINE DANCES:
Keep the line that best describes the way you feel about line dance songs.
Delete the other two lines or type your own opinions about line dancing.
--Big fun! I'll do the Wobble anytime.
--Cheesy. Play line dances only if guests request them.
--No line dances, even if guests beg for them!
Delete the line dances you hate and keep the line dances you'd like to hear me play.
--The Cupid Shuffle
--The Cha Cha Slide
--The Turbo Hustle
--The Chicken Dance
--The Hokey Pokey
--The Bunny Hop
ROCK 'N' ROLL SURVEY:
In general, there isn't much room to play a whole lot of rock because it tends to clear
the dance floor. However, I'm a big rock fan, and many of my brides love rock. If there
are moments when dancers are resting, I can sneak in a couple rock songs and monitor
how well the audience responds.
Enter A, B, or C to indicate the categories you like to hear. You may choose as many
categories as you like. I'll avoid playing songs from categories you don't choose.
A. 1960s & 1970s Psychedelic & Hard Rock: Led Zeppelin, The Rolling Stones, Aerosmith,
Black Sabbath, Deep Purple, Alice Cooper, the Doors, Jimi Hendrix, Cream, Jefferson Airplane
(These bands definitely won't offend the older crowd. This stuff will fly at weddings.)
B. 1980s Hard Rock: AC/DC, Van Halen, Def Leppard, Guns 'N Roses, Poison, Bon Jovi
Motley Crue, Warrant, Great White, The Scorpions, Ratt, Quiet Riot, Ozzy, Queensryche,
Ronnie James Dio, Iron Maiden, Megadeth, Metallica, Pantera, Motorhead, Judas Priest.
(Upbeat selections from the first ten bands tend to go over well at weddings. I would avoid
the last ten bands listed. They don't go over well.)
C. 1990s Hard Rock and Industrial: Godsmack, Tool, Korn, Nine Inch Nails, Allice in Chains,
Marilyn Maonson, White Zombie, Danzig, Ministry, Fear Factory. (I love this stuff as much as
anyone, but it doesn't go over well at weddings. I don't recommend it.)
D. Modern Punk: One Direction, Green Day, Blink 182, Fall Out Boy, All American Rejects,
Paramore, Good Charlotte, Sum 41, Simple Plan, The Used, Rise Against, My Chemical Romance.
(Punk groups such as The Misfits and Rancid would be highly offensive to most guests.)
